This two-sided tag has your child's allergies listed on one side and on the other are instructions on what should be done in case of an emergency along with the child's name and your contact info.

The tag is professionally printed on thick, hard plastic.

1—Add this listing to your cart or click "buy now".

2—At checkout enter your tag information in the personalization box. Please include the food allergens (see the 3rd picture above for examples), and all info to be printed on the back of the tag; child's name, instructions in case of reaction, parent's phone #s, etc. **If you don't see your child's allergen message me and I will add it!

3—Complete check-out.

4—I will create a proof of the tag and send it to you via the Etsy convos messaging system within 1-2 BUSINESS DAYS. Once I receive your approval the tag will be completed and shipped out.
